Open surgery for the frontal sinus is greatly diminishing with advances in endoscopic
techniques and tools. However, open surgery is still indicated in several instances,
recalcitrant frontal sinusitis in high risk patients, osteomas, and soft tissue tumors
of the frontal sinus. The rate of complications is low. Mucocele formation and forehead
paresthesia are the most common; each less than 10%. Osteoplastic flaps with and without
fat obliteration may have limited indications, but it can be very useful in the right
patient.
